Shyona casts out those who betray their families. The court strips the name, strikes it from every record, forbids it ever to be spoken, and sends the condemned west into the Hills of Dolor to die. For most of the realm's history the sentence was reliable, because the desert finished what the court began. The Refusal is the name later given to the first time it did not.
Generations ago, a scatter of name-stripped exiles out of Shyona's honor-bound Shontobi houses, and the servants who had followed their masters into disgrace rather than abandon them, reached the deep wells at the bottom of the Hills of Dolor still breathing. A name-stripped exile who lives that long can do one of two things. They can go on as a ghost of who they were, hoarding the forbidden name in private, waiting for a recall their own law makes impossible. Or they can let it go. The first survivors let it go. They buried their names with the dead they had buried on the road, agreed to raise the next children in common with no house and no lineage to inherit, and took the Shontobi word for the nameless, half-people, those without identity, and wore it on purpose. They refused to die. They also refused to go on being the people Shyona had unmade. That double refusal is the thing they built themselves out of, and it is why their descendants say "nameless" the way other peoples say the name of a homeland.
The first wells lie at the bottom of the Hills of Dolor, where the exile road ends. The names of the founders are not carved there. Nothing is. By the time anyone thought to mark the place, the people who had survived it had already decided that to mark it with names would undo the only thing they had agreed to become.
Every institution of the Nameless reenacts the Refusal. The words spoken over each newcomer at the wells, to leave the name in the dust and drink and be no one, are the first choice performed again on every arrival. A people who keep no lineage and ask no one's past are a people who have agreed, over and over, to make the same decision the founders made. The one thing that cuts against it is the Goldwatch's Tally, the secret ledger of exactly the names everyone is supposed to have shed, and the Tally is the standing argument over whether the Refusal was a liberation or a wound a people chose to carry. Keshwindi has never settled that argument. It only repeats the choice each time the wells take someone in, and it has never once agreed aloud on what the choice cost.
